Empire Life Link may help individuals identify healthcare access, food, housing, transportation, and other community-support needs. Participation in screening is voluntary.

Information Collection
Screening may involve personal information, Medicaid coverage details, and health-related or social-needs information. Sensitive information should be collected only through an appropriately secured and approved process.

Referral Authorization
Sharing identifiable screening information with SOMOS or another healthcare or community organization may require separate, specific authorization or another valid legal basis. A general website inquiry does not automatically authorize sharing sensitive information.

Consent Withdrawal
Individuals may ask to withdraw voluntary consent for future activities, subject to applicable legal requirements.

Program-Specific Requirements
If screening is conducted through an authorized New York Social Care Network workflow, applicable NYSDOH and program-specific screening and information-sharing consent requirements must be followed.
